I looked at this again and I don't see any possible way the trigger linkage yoke would affect this. In fact, as I look at the action more closely, I'm pretty convinced that this is my issue. I'm going to go the shim route.

There are two very small screws that’s hold the side plate on. If you tighten the forward screw too tight, more than just snug, the gun won’t index correctly.

Thanks guys. I did experience the weirdness with the side plate. Noted! But even with the side plate off, it did not index right.
I decided to "shim" with a drop of 2-part epoxy and then file it down to the correct depth. It seems to have worked like a charm! Hopefully it will last!
