Tomahawk breech jaws

It’s a custon cut tad oversized
6x1 With a fitted .250 center stem bolt
I’ve taken hundreds apart.
never striped one unless you used a different screw than the actual unit.
Gonna be a potential issue if not done correctly as the forks will not squeeze equally
then you get breech sway

1 what happened.
2 how did this happen ?

There are several great mechanics over there. Get it to one of them instread of further damaging it.

I think I did it trying to get the bolt in before I noticed the obstruction (the barrel being turned)

run a 6x1 tap through it and you will be fine. The bolt is the tad oversized. If starting threads are crushed a tad use a needle file to re shape. test fit it with no breech in between.
That gun looks like it’s been soaked in acid. 😳
Good, but, I'm thinking
I would suggest a thread chaser first. They swage the existing threads back in place instead of cutting away and thinning the thread. But, you know that sometimes this is the stuff that gets people in over their heads. Separates the men from the boys ;)
